  • Are you engaging in any new self harm or hurtful way towards yourself or others?

  • Have you been feeling more angry or irritable with yourself or others?

  • Are you avoiding contact with others or procrastinating more?

  • Are you feeling helpless or hopeless, and feel like you are losing your mind?

  • Are you feeling like this is too hard or that this is all your fault and no place seems safe?

  • Are you feeling anxious and having panic attacks or intrusive thoughts?

  • Has anyone recently expressed any worries for you or your well-being and safety?

  • Have you experienced a health problem or financial loss or grief/loss or death recently?

  • Have you or someone you love, experienced any situation of abuse recently?

  • Are you feeling preoccupied with feelings of loss or self blame or other emotions/thoughts?

  • Have you noticed significant changes with sleep or eating or having difficulty sitting still?

  • Are you feeling always tired or have no energy to go out or to work?

  • Have you lost or gained weight?  Are you preoccupied with your body changes?

  • Does your whole body or your head hurt a lot more?  Are you feeling nauseous/breathless?

  • Are you having difficulty with concentration or task completion?

  • Are you worried about any family member or significant others with the above concerns?
